Order and Payment Information

Setting up account

Setting up an account is required if you are the new customer. To do so, please provide the following information by email to sales@anisyn.com or fax to 1-269-372-3397

1. A signed purchase order form
2. Your shipping and billing address
3. Contact person's name, phone and fax number, and email address
4. FedEx or UPS account number if applicable

Order Methods:

1. By fax: If you have a purchase order with your purchasing department, please fax it to us at
   1-269-372-3397. You may use your own order form, or download the Order Form we provide
   (in pdf format).
2. By phone: You may call us at 1-269-372-8736 to place your order.
3. By email: You may also send your email order with filled order form to sales@anisyn.com

Payment Methods:

We accept check, money order, wire transfer, and credit cards.

1. Check or money order. Please make it payable to Anisyn Inc., and send it to our mailing address.
2. Wire transfer. Please contact us for our bank information.
3. Credit card. We accept Visa, Mastercard and American Express. Please call us (1-269-372-
   8736end_of_the_skype_highlighting) or fax to us (1-269-372- 3397) and give us the following
   information: (1) Card type, (2) card number, (3) expiration date, (4) card holder name and address,
   including city, state and zip code. You may use our credit card payment form (in pdf format).
